Fees

Our providers are considered out-of-network. We’re happy to provide a superbill upon request, which you may submit to your insurance for potential out-of-network reimbursement. Our billing assistant, La Shawn Farrish, can assist you with this process.

To obtain a claim form to submit to insurance:

• Log in to the patient portal.
• Navigate to the Billing tab.
• Click on the Ledger tab.
• Select the claim number for the relevant date(s).
• Click the plus icon next to the claim number and check each box for that date.
• Click the Print button to obtain your claim form.

You have the right to receive a Good Faith Estimate of the cost of your care. We are happy to provide one — just ask your doctor or our office staff.

Payment

Payment is expected at the time of service. Accepted methods include:
